The Recipe
3 Magic Amulets -> 1 Magic Ring
Works in all modes (single player, bnet, open bnet, multi)
Specifics
This cube recipe consumes three magic amulets, and produces a new magic ring. There is a similar recipe for 3 rings -> 1 amulet, but it is not covered by this guide.
The resulting item level of the new ring is determined by this formula: multiply character level by 75%, round down if you get a decimal.
ilvl = int(.75 * clvl)
Examples:
clvl = 50, ilvl = 37
clvl = 80, ilvl = 60
clvl = 98, ilvl = 73
clvl = 99, ilvl = 74
This is important because ilvl is used to determine which affixes (prefix and suffix) can appear on a magic item. Every affix has a minimum ilvl: if the base item is not ranked highly enough, the affix simply cannot appear. (This helps to prevent Quill Rats from dropping Ring of Attract, requiring level 60 to equip).
Independently of the formula above, the new ring will have one suffix, one prefix, or one of each according to these probabilities:
50% Suffix Only
25% Prefix Only
25% Both
That translates to a 1-in-4 odds of rolling a 2-modifier item - you know, the good stuff : )
This recipe is interesting for two reasons: first, the item level of the input jewelry is unimportant: the only thing that matters is the level of your character. So, gather up every crap magic ring and amulet you run across, and hang on to it for your high level to craft with. Failed crafting attempts can also be recycled into the next recipe, making this a very convenient and easy way to produce some useful items.
It is also interesting because there are no additional ingredients. You don't have to hunt down gems or jewels or runes or whatever. Just chuck in three amulets and go.
Rare, unique, set jewelry can't be used in this recipe.
Where do I get materials for crafting?
Lots of monsters have the ability to drop jewelry, so many will spawn in a longer game. Many players ignore blue rings and amulets in a game, or quickly ID and then drop them back on the ground. You should be able to scavenge quite a lot of rings this way.
If a player grabs your amulet and doesn't drop it, follow him back to town next time. He's likely to ID it, disapprove, then sell it to a merchant. You can simply buy it back for yourself. If it didn't carry charges it's probably cheap.

What can I not get with this?
Because maximum ilvl of rings generated with this recipe is 74 (using a level 99 character), some affixes will be excluded from the list. Here is a table showing affixes which cannot be crafted using this recipe.

Okay, but what do I actually WANT to get?
First, you want an item with _both_ prefix and suffix. Multi-affix does not increase the required 'to equip' level - it simply uses the higher of the two affixes - so shoot for two when you can get it, there are no drawbacks aside from the 25% odds to get it.
Next.. Of the available prefixes, these are the most valuable:
- Platinum (+101-120 AR)
- Bahamut's (+91-120 Mana)
- Scintillating (+12-15 Resist All)
- Fortuitous (+11-15% MF)
Lesser valued suffixes could be perfect single-element resists, but only if combined with a good suffix.
- Garnet (+21-30 Resist Fire)
- Cobalt (+21-30 Resist Cold)
- Coral (+21-30 Resist Lightning)
- Jade (+21-30 Resist Poison)
Next, the list of most valued suffixes:
- Titan (+16-20 Str)
- Apprentice (10% FCR)
- Mammoth (+31-40 Life)
- Wizardry (+16-20 Energy)
- Fortune (+16-25% MF)
Lesser valued suffixes are these second-tier affixes, where the best version can't be crafted - only gambled or dropped.

- Locust (5-6% Life Leech)
- Wraith (4-5% Mana Leech)
Pretty much any combination of Good Prefix + Good Suffix makes for a very nice ring. Exceptions would be useless cross-affixes, like a Platinum Ring of Wizardry or a Platinum Ring of the Apprentice - adding AR to caster rings is useless. Bahamut's Ring of the Locust may be of some use to non-caster classes: life leech + mana to use skills is OK, but obviously excludes this usage from casters who have no need for LL.

Fortuitous Ring of Fortune
11-15% Better Chance of Getting Magic Item / 16-25% Better Chance of Getting Magic Item
This combination is very rare indeed, personally I have found only one ring with this pre and suffix in all my time playing Diablo 2. Obviously the closer you get to the coveted possible 40% total of this combo the more valuable the ring will be, anything above 35% is worth gazzilions with the perfect numbers of 15 and 25 on same ring being nearly impossible to appraise. If you are a really serious treasure hunter such a ring is a must and will in most cases be used on characters built purely for the purpose of Magic Finding. Some might argue that a few % mf is of little consequence, but purists know the true value of perfect items.

+91-120 To Mana / +31-40 Life
Any spellcasting character class will find this combination invaluable, with bonuses to both mana and life reserves. Even some mana intensive melee characters can benefit from these bonuses, at least until they get a high enough source(s) of leech. Its worth mentioning that similar pre and suffix combinations exist on charms which makes this ring useful but less than stellar.
Bahamut's Ring of Wizardry
+91-120 To Mana / +16-20 Energy
For a great mana bonus this is the optimal combination on a magic ring. Try wearing some items with the mod of % Increased Mana besides this ring for a really huge boost to your mana reserve (like a Stone of Jordan).
Scintillating Ring of the Mammoth
+12-15% To All Resistances / +31-40 Life
Of special interest to some characters might be a combination of added resistances and life, two things which any Hardcore player can't have enough off.
Bahamut's / Scintillating Ring of the Apprentice
(+12-15% Res All OR +91-120 Mana) / 10% Faster Cast Rate
For being such a low-level affix, Of The Apprentice is actually quite useful, since FCR is so needed for many builds and this is the only FCR affix available on rings. Alone it's not valuable, but paired with one of these two prefixes it becomes an excellent caster ring. Bahamut's is obviously ideal for characters that burn through mana, while Scintillating provides a resist-all boost to help survive in Hell mode which anyone could benefit from.
What level should I use to craft?
There are two approaches to take here.

The first: simply use your highest level to roll everything and see what comes out. The advantage is that high levels have access to a wide array of affixes, making all combinations available. The disadvantage is that there is a bit of crap available with high ilvl requirements (Attract and Confusion Charges, + to Min / Max / Poison / Fire / Lightning damage (all too low damage to be useful), Replenish Life (too slow to be useful), and the Chain Lightning on Attack / Nova when Hit) which you could potentially roll as well. Carefully inspecting your level first may give you the best chance.The second: pick what you want, and use a character whose level ranges from (min CLVL to roll) to (min CLVL to roll next highest affix, -1). This deserves an example:
Suppose you're gunning for a Fortuitous Ring of Fortune. Checking the table, we see the minimum CLVL to roll Fortuitous is 16, while the min to roll Of Fortune is 56. So we need at least a level 56 character to get it.
The next CLVL TO ROLL after 56 is 60: Bahamut's, Shimmering, of Blight, and the final Chain Lightning on Attack become available. If you try to roll rings with a level 60+ character, you're making these additional 4 affixes available, which dilutes the pool a bit and makes it harder to get exactly what you want. You should thus roll for Fortuitous Ring of Fortune with a character level 56 - 59 for the greatest chance of success.
Without doing all that work, if you just want to maximize your chances of getting something useful, follow these general guidelines:
You might want to wait to start rolling until you hit level 56, when some of the best prefix / suffix combinations can be generated - see 'what do I actually want', above. Of course you can roll sooner if you wish, but the best you can do is get second- and third-string affixes.. or Platinum Ring of (the Apprentice / Wizardry), a combination with very questionable usefulness.
Don't bother rolling with character level between 64 and 67. Confusion charges (64) and Shock (67) are useless suffixes that start appearing here. Tiger, at clvl 68, is beneficial.
Do not roll in clevel range of 79 - 89. Excellence and Of Attract starts showing up at 79 and 80 - both crap. 90 is a good time to roll again because Scintillating becomes available.
